Clinical Studies on Acute Pyelonephritis

Abstract
|
|
|
A clinical study was carried out on 351 cases of acute pyelonephritis who were admitted to the Department of Internal Medicine of Busan National University Hospital and the St. Bendict Hospital during past 6 years, from January, 1976 to December, 1981.
The results obtained as follows:
1. The sex distribution was 42 cases in male and 309 cases in female, and the ratio of male to female was 1 : 7. 35.
The highest age incidence of acute pyelonephritis was found in third decade(38. 2%), followed by forth decade(27. 9%), and 69.3% of cases oocured in reproductive female cases in 20-40 years of age.
2. The predisposing factors recognized in 38.2%, and they were pregnancy(10. 3%), typhoid fever (7.4%), obstructive uropathy(6.0%), instrumentaion of urinary tract(4.8%), diabetes mellitus. (4.3%) and pelvic inflammatory condition(3. 4%) in the order of frequency.
3. The common clinical symptoms were fever and chill, followed by costovertebral angle pain and knocking pain, urinary frequency, vomiting, nausea, and headache in the order of frequency.
4. Microorganisms were isolated by urine culture in 197 cases(56. 1%), and the most common organism was E. coli(73. 1%), followed by colifrom- bacilli_ and. staphylococcus, klebsiella(4. I%), salmonella (3. 6% )and enterococcus(3.I%) in the order of frequency.
In male, the most common organism was staphylococcus(40. 0%) on the other hand in female, was E. coli(78. 5%).
5. In sensitivity tests of the organisms to antibiotics, kanamycin was the most sensitive(72. 6%) to the whole organisms, followed by gentamycin(66.5%), ampicillin(47. 7%), cephaloaporin(46. 7%), minocyclin HcI(42.6%), and in sensitvity to E. coli, kanamycin(77. 8%), followed by gentamycin (66.7%), minocyclin Hcl (49.3%), ampicillin(47.9%) in the order, and on the other hard staphylococcus was more sensitive to cephalosprin(81. E%), followed by ampicillin(E3.6%) and gentamycin(54. 5%) in the order.
